Disclosures to Service Providers

We may disclose the information we collect, including patient health information, to individuals and companies that provide services on your behalf. Such individuals and organizations may be:

No Uses or Disclosures of Patient Health Information for Other Purposes

We may de-identify your information, including your patient health information, and use and disclose such de-identified information for any purpose. Any patient health information we de-identify will be de-identified in accordance with the current Standards for Privacy of Individually Identifiable Health Information promulgated by the U.S. Department of Health and Human Services pursuant to the Health Insurance Portability and Accountability Act of 1996.

Our Security Procedures

We maintain procedures and technology designed to prevent unauthorized access to your information, including patient health information. We maintain physical, electronic, and procedural protections in accordance with applicable standards to protect patient health information. We restrict access to patient health information to our employees, our subcontractors and agents, your service providers for legitimate business purposes to assist in providing products and services to you. Our employees, subcontractors, and agents are subject to disciplinary action for violations of our Privacy Policy.
